The Minden site of HHHS currently includes ambulatory clinics (physiotherapy, bone densitometry, DEN/Gain, and outpatient x-ray by appointment) and is the location for the Hyland Crest 62-bed long-term care facility as well as program offices for supportive housing and diabetes education, which are located on the second floor in the community health area. The Geriatric Assessment and Intervention Network (GAIN) offices are adjacent to the site at 8 Winchester Street.
